  • SMART Recipient Scholarship Funds Transfer Form

  • SMART policy allows Recipients to transfer funds to a qualifying family member. This form is to be used for the transfer request and must be signed for EACH request.

    Scholarship funds received from another family member via transfer or directly from SMART (active recipient bonus scholarships or grants) are NOT transferrable. Please see the SMART Policy Manual, Chapter 4, item 5, for more information on transfers.

  • By signing this form, I affirm that I am 21 years of age and that the individual I am transferring funds to:

    1. Has been a USBC member for at least four (4) years.
         a. USBC membership can be either youth, adult, or any combination thereof.
         b. Does not have to be a current USBC member.
         c. Does not need to be consecutive years.


    2. Can use the funds within eight (8) years of their high school graduation.

    3. Agrees to the transfer.

  • I understand once I have funds transferred to another individual, I have no claim on these scholarship funds.

    If I am determined to have received cash for these funds or submitted this application or other documents in an effort to defraud SMART, I understand this could result in my suspension by the United States Bowling Congress, and I may have to pay the funds back and/or forfeit any/all remaining funds in my SMART account.
